Due to the Covid-19 Pandemic, we ask that you consider minimising the number of family members/support persons coming with you to this appointment.

We apologise for any inconvenience this may cause you. 

What to bring with you to your appointment?

  • Your Medicare card

  • Any health fund details

  • Any pension or health care card details

  • A valid referral from your GP

  • Any relevant imaging of your breast (including mammograms, ultrasounds, MRI and other scans)

  • Any blood tests and pathology reports

  • Any reports of biopsies or previous breast surgery results

  • A list of your current medications

  • If you have had surgery before it is helpful to bring any information you have about your previous surgeon and surgery

  • If you have any significant medical conditions and see other specialists or doctors for these, please bring as much information as you can as it may be relevant in planning for surgery.

  • If you cannot speak English well and need help understanding or explaining yourself, please bring along a relative or friend who can translate for you. If you need a translator booked for you, please tell us when making your appointment.

  • Bring a relative or friend for support.

  • A list of any questions you may have.

Are you having surgery with A/Prof Mak or Dr Susannah Graham?

Following your initial consultation with A/Prof Mak or Dr Graham, our staff will assist you to plan for your surgery.

Important Information

You will need a referral from your General Practitioner. This can be faxed through or you can bring it with you. You are still able to attend without your referral, however you will not be able to claim your Medicare rebate for the appointment. 

Our cancellation/no-show (DNA) policy

Our practice is committed to providing all of our patients with exceptional care. When patients do not show up or cancel without giving enough notice, they prevent another patient from being seen sooner.

If you need to cancel your appointment, please call us at (02) 9519 8525 before 12pm the working day prior to your appointment so that the staff can allocate your spot to another patient needing an urgent appointment. Cancellations received after this time, as well as no-shows, may attract a cancellation / DNA fee.

The information from your registration form, health questionnaire and aforementioned documents will be recorded and used for the primary purpose of providing care for your presenting problem, including for administrative and billing purposes.  It may also be disclosed to other health care professionals or relevant parties with your consent.
